51 year old male sustained a direct blunt force injury to the shoulder at work resulting in a Type 4 ACJ dislocation. Surgery was performed 3 months later. Final follow-up 3 years after surgery showed good results with a Constant score of 91 (0-100) and a UCLA score of 27 (0-35). Due to continued local pain in the AC joint that failed to resolve with a cortisone injection he underwent an arthroscopic distal clavicle excision. The second surgery resolved his pain issue. Arthroscopic evaluation at the time of the second surgery showed evidence of scar tissue filling in the coracoclavicular interval.
